Most people even therapists concern mainly the range of "Output Intensity", "Pulsed Frequency (Rate)", "Pulsed Duration" and "User Friendiness" in evaluating a TENS or NMES device. The key factor of "Effectiveness of Penetration of Electrical Current" is usually overlooked. In TENS or NMES treatment, electrical current has to go through skin before it can reach and stimulate nerve or muscle. Normal human skin has a resistance of about 1,500 ohm and people with poor skin condition like dried skin of elderly person may have a resistance over 2,000 ohm. Most electrical stimulators in the market especially TENS devices have current load of 500 ohm; which means they can maintain output intensity at resistance up to 500 ohm. A maximum output setting of 100 mA of 500 ohm load may result only 20-30 mA after penetrating human skin. Increasing output intensity can be a way of compensation but it will cause discomfort on skin and may cause irritation. Besides, accommodation will soon make these 500 ohm load devices becomes less effective.
